Proctor Application Form - for Oregon students only

Self-Paced Online and Correspondence students enrolled in our Basic Tax Course who live more than one hours drive from Portland may apply to have their Mid-Term and Final Closed-Book Exams proctored.  Proctors must be approved by the School. 

If you want to have your Mid-Term or Final Exam proctored, you and the proctor you select must complete and submit the Proctor Application Form to Pacific Northwest Tax School. Upon approval, Pacific Northwest Tax School will deliver the Closed-Book Mid-Term Exam and/or Final Exam directly to the Proctor. You will take the Exam at the Proctor's location. Upon completion, the Proctor will return the test to the School for grading.

Examination times and locations for our Oregon students only

You must call the school to  schedule your examination.  Any student showing up without having set an appointment will not be allowed to sit for the exam. 

These are 3.5 hour long multiple choice, true/false and mini problem exams, days offered, locations  and start & end times are shown below.  You will be provided with the Oregon Board of Tax Practitioners Reference Sheet and any tax forms you will need with each exam you take.

What you need to bring to your exams:

Midterm Exam: 

  • photo ID
  • non programable calculator
  • several pencils with erasers. 
  • NO tax resources such as IRS Pub 17, State or Oregon 17 1/2 or any other printed tax resource will be allowed into the Midterm Exam.

Final Exam: 

  • photo id
  • several pencils with erasers
  • non-programmable calculators.
  • acceptable source documents - documents must be official publications or copies downloaded from the official Web sites of the IRS and the Oregon Department of Revenue

What you may not bring into the exams:

  • Cellular phones, computers, laptops, Ipads or other communication devices
  • Ipods, MP3 Players or other recording/listening devices
  • Hand written notes or other unapproved study materials
